Assist clients with ADL's 2.
Implement individual goals as scheduled and complete the data sheets accurately and fully.

Identify, report, document and provide individual protection in the event of an occurrence, reportable or serious reportable incident or allegation of abuse.

Uphold and protect the rights of all individuals.
Maintain health and safety of all individuals.
5.
Foster a person -centered atmosphere conductive to the growth and development of individuals.
6.
Transports individuals in agency vehicle for medical appointments and recreational activities.
7.
Assist individuals in preparing meals or cook meals for the individuals.
8.
Assist individuals with personal grooming and hygiene.
9.
Have sufficient knowledge to fulfill their role as it pertains to service plan.
10.
Promote health and wellness of clients through health maintenance and prevention strategies, knowledge of general health and safety rules, first aid and emergency procedures.
11.
Know organizational mission, policies and procedures, understand their role and responsibilities within the organization, and respect the organization in a responsible manner.
12.
Advocate for clients as mandated reporters of abuse, neglect and maltreatment.
13.
Be aware of the requirement for documentation within the program and organization as a whole and manage those requirements in a thorough and efficient manner.
14.
Enhance the ability of the client to lead a self-determined life providing the support and information necessary to build self-esteem, assertiveness and independence.
15.
Knowledge of formal and informal resources and supports available in their community and are skilled in assisting the clients to gain access to such supports.
16.
Assist clients in building/strengthening skills necessary to develop and maintain friendships and other personal relationships.
17.
Utilize the range of effective communication strategies and skills necessary to establish collaborative relationship with clients, families and resources.
18.
Perform job duties and interact with clients and co-workers in a professional manner at all times.
19.
Attend and successfully complete all required trainings.
20.
Perform such other duties as assigned.
21.
Adhere to agency and program policies, procedures, conduct and rules including Agency Compliance Policies and Procedures.
22.
Stay beyond working hours in necessary situation.
23.
Participate in special assignments or projects as dictated by the need of the program.
24.
Maintain confidentiality of all patients; consult with Supervisor prior to releasing confidential information; obtain release of information from patient prior to any disclosures of patient information.
